AQ-H02 is the original five-gate independent witness hold and release form for mine ground support. It demands witnessed proof for pre-entry scaling, traceable support tests, barricaded safe access, changed ground and mine engineer hold-point approval.
WHEN TO USE THIS UNDERGROUND SAFETY RECORD
Use this source before handing back a mine support face or transferring to another crew. Enter MINE / AREA / LEVEL, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR and relate all five holds to the current geotechnical design and work order.

GROUND SUPPORT FAILURE AND OPERATOR RELEASE
The RELEASE CONDITION explicitly requires all applicable holds PASS, approved work-order traceability and witnessed corrections. Only a named mine official can grant return-to-service; a contractor signature does not release unsupported ground.
DEFECTS, WITNESSES AND FINAL ACCEPTANCE
The original records contractor supervisor and host mine authority signatures where required, and Date / release reference. The mine official's decision is the only valid release.
