271 / AQ-F01 — HIGHWALL & BENCH TECHNICAL HOLD AND MINE WORK RELEASE

AQ-F01 is AVC's original one-page editable Word FIVE-GATE technical HOLD and work-release record for highwall, bench, scaling and geotechnical monitoring support. The FIRST gate requires a current hazard map identifying actual unstable areas and monitoring alerts, followed by explicit mine geotechnical engineering access approval.

THE HIGHWALL RISK AND APPROVAL THAT THIS RECORD ADDRESSES

A work order to calibrate a prism or survey a pit crest is not clearance to enter a potential rockfall zone. The mine's radar settings, TARP trigger levels and post-blast/rain ground condition can make an earlier access authorisation invalid. AQ-F01 creates a source-specific release gate instead of allowing contractors to assess slope safety on appearance.

FIVE ORIGINAL EVIDENCE GATES

1. Current mine geotechnical hazard map identifies unstable zones and monitoring alarms. 2. Mine geotechnical engineer authorises drilling, surveying and access near pit crest/toe. 3. Mine owner controls radar, prism and crack instrument status and thresholds.

4. TARP defines warning, withdrawal and restart rules. 5. Rain, seismic, dewatering, blast damage and progressive failure assessed at each shift.

SOURCE FORM FIELDS, EVIDENCE AND CONTROLLING RELEASE

The source has precisely FIVE HOLD TRIGGER / EVIDENCE GATE rows, each with REQUIRED EVIDENCE / VERIFICATION and RESULT / REFERENCE / WITNESS. It does not contain a seven-row slope movement table. Record the actual current map/version, geotechnical engineer's crest/toe approval, sensor evidence, TARP and shift condition inspection.

The printed RELEASE rule requires all five gates verified, defects closed and a valid work release entered by an authorised mine official. CONTRACTOR SUPERVISOR / SIGNATURE and MINE RELEASE AUTHORITY / SIGNATURE plus DATE / RELEASE REF separate checking from approval.
