268 / AQ-C02 — HIGHWALL & BENCH CHANGED CONDITIONS, WEATHER AND EXPOSURE

AQ-C02 is an original editable seven-row Word highwall CHANGED-CONDITIONS workface checklist. It requires contractors to reassess mine-owned surveillance thresholds, TARP triggers and pit access after rain, blasting, dewatering, seismic activity or new evidence of slope movement.

THE HIGHWALL RISK AND APPROVAL THAT THIS RECORD ADDRESSES

The most dangerous change to a highwall may be a trend that is not obvious visually: a prism displacement, new seepage or a newly exceeded radar alarm. Continuing a previous permit or moving a barrier without a current mine engineer review can expose crews to a fall-of-ground. AQ-C02 forces the issue back through surveillance and written release.

SEVEN ORIGINAL WORKFACE SAFETY CONTROLS

1. Mine-owned radar, prism, crack instruments and threshold changes controlled. 2. TARP sets warning, withdrawal and restart thresholds. 3. Rain, seismic, dewatering, blast and progressive ground changes assessed. 4. Fall-of-ground barriers and no-go boundaries remain intact.

5. Recorded trends and alarms escalated promptly to geotechnical authority. 6. Re-entry and resumed work depend on written/georeferenced release. 7. Current hazard map identifies mine instability zones and alarms.

SOURCE FORM FIELDS, EVIDENCE AND CONTROLLING RELEASE

The printed table uses Y / N / N/A and EVIDENCE / DEFECT / ACTION for each of SEVEN checks. It begins with instrument-status/threshold management and finishes with the CURRENT geotechnical hazard map. There is no built-in rain gauge or sensor chart; cite real trend, TARP revision and access records in the evidence lines.

The printed HOLD requires stopping for missing clearance or critical protection and reassessing changed conditions. Contractor supervisor and mine release authority sign separately with DATE / RELEASE REF; neither weather observation nor checklist completion authorises entry.

BUY THE INDIVIDUAL FORM OR COMPLETE MINING SYSTEM?

This exact original editable purchaser-licensed DOCX is ALREADY INCLUDED in the 386-document Surface Mining Checklist Package and in the full Contractor H&S File, together with core safety and specialist trade controls. Buyers of either full collection already receive this form without buying it separately. All live geotechnical thresholds, TARP responses, access permits and re-entry releases remain subject to mine authority.
