262 / AP-T01 — BLASTING EXCLUSION INSPECTION, MEASUREMENTS AND RE-ENTRY ACCEPTANCE

AP-T01 is the original editable Word seven-row inspection and acceptance-evidence register for mine blasting support and post-blast re-entry interfaces. It gathers documented mine-controlled exclusion maps, headcounts, communications, safe plant positioning, zone conditions and authorised clearance references.

WHY THIS ORIGINAL MINE BLAST CONTROL MATTERS

A post-blast area may be visually quiet without being cleared for entry. Correct acceptance depends on the blast authority's written release, geotechnical checks and a traceable headcount/issue record. Contractors must not mistake their own measurements or observations for the power to release an explosive exclusion zone.

THE SEVEN ORIGINAL INSPECTION / EVIDENCE ROWS

1. Exclusion boundaries/refuge routes received with current controlled blast map. 2. Crew, vehicle and subcontractor headcount completed before evacuation. 3. Radio, warning siren, blast timetable and boundary marshal test references available. 4. Contractor machinery parked and made safe outside flyrock/damage zone.

5. Identify post-blast fumes, dust, ground damage and unexploded-charge risks. 6. Re-entry withheld until mine blast authority and geotechnical checks release the zone. 7. Suspected misfire or flyrock damage reported without touching remnants.

ACTUAL SOURCE EVIDENCE, HOLD AND MINE RELEASE

Identify MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. The printed table has MINING-SPECIFIC ITEM / EVIDENCE TO TRACK, DOCUMENT / RESULT / MEASURED VALUE and DATE / AUTHORITY / STATUS for each of seven rows. The source requests blast-area log, headcount, authorised clearance, re-entry release and issue register. Enter actual IDs, decisions and measured/assessed evidence; NEVER fabricate misfire clearance.

Use OUTSTANDING DEFECT / NCR / REINSPECTION / RETENTION FILE REF for outstanding evidence. Contractor supervisor and MINE RELEASE AUTHORITY signatures are separate, with DATE / RELEASE REF. Neither the form nor contractor test results grant entry.

INDIVIDUAL EDITABLE WORD OR THE COMPLETE MINE COLLECTION?

This exact original purchaser-licensed editable DOCX is ALREADY INCLUDED in the 386-document Surface Mining Checklist Package and the complete Surface Mining Contractor Health & Safety File. Buyers of either larger set receive this form alongside wider site and specialist controls, rather than purchasing it separately. Blasting, explosives and site-clearance authority always remain with the mine.
