260 / AP-R03 — BLASTING SUPPORT WORK RESULTS AND MEASURED EVIDENCE

AP-R03 is the original editable Word seven-row work-results, measured/assessed evidence record for contractor operations next to mine-controlled blast zones. Its first item explicitly retains firing and clearance authority with the mine blast officer, while the contractor records workpack results and references.

WHY VERIFIED POST-BLAST EVIDENCE MATTERS

The mine may change the blast timetable or refuge boundary while contractors perform unrelated works. Without updated blast-zone maps, complete evacuation headcount, siren/radio checks and the current formal all-clear, the contractor's claim that its task is complete is not enough to prove safe access.

SEVEN ORIGINAL WORKFACE EVIDENCE ROWS

1. Blast officer retains explosives, firing and all-clear; contractor scope defined. 2. Controlled blast exclusion maps and refuge-route revisions received. 3. Crew, vehicles and subcontractors accounted for before evacuation. 4. Radios, sirens, blast timetable and boundary marshals tested.

5. Contractor equipment parked beyond damage and flyrock exposure. 6. Post-blast fumes, dust, unstable ground and unexploded-charge hazards recognised. 7. No re-entry before authorised blast release and geotechnical checks.

ENTER REAL EVIDENCE, WITNESS AND MINE AUTHORITY

Fill MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. The source specifically calls for blast-area log, headcount, authorised clearance, re-entry release and issue register. Enter actual work order, document ID, result and authority/status against the seven printed subject rows; never invent an all-clear or assume a local blast event was authorised by this record.

Use the original OUTSTANDING DEFECT / NCR / REINSPECTION / RETENTION FILE REF for unresolved issues. Contractor supervisor and MINE RELEASE AUTHORITY / SIGNATURE plus DATE / RELEASE REF remain separate; verification does not authorise re-entry.

THIS INDIVIDUAL WORD OR A COMPLETE SAFETY FILE?

The original editable purchaser-licensed DOCX is ALREADY INCLUDED in the full 386-document Surface Mining Checklist Package and the complete Surface Mining Contractor H&S File. Buying either comprehensive set includes this form alongside management and specialist annexures; buyers of the full set need not buy it individually. Explosives, firing and clearance stay under host-mine control.
