257 / AV-R02 — Process Exception, Change & Closeout Register

WHERE AND WHY IT IS USED

Use this controlled exception tracker when refinery support services, stores, waste handling or digital work develops a deviation from the accepted work order or needs a documented change. An unresolved plant item, changed material or IT access restriction can outlive the working shift; the register keeps that decision visible until the facility authority has dispositioned it.

THE ACTUAL PRINTED CONTROL FIELDS

Each working row has four exact headings: PLANT ITEM / WORK ORDER; DEVIATION / HAZARD; DECISION / CONTROL / EVIDENCE; and RELEASE AUTHORITY / DATE. Record real identifiers and references rather than a general statement that the issue was handled. The top fields tie the record to the facility, unit/equipment, work order/PTW reference, contractor and shift. File the relevant inspection, permit, signed test or decision evidence behind the register.

HOW TO COMPLETE AND SIGN IT

Open a line when a process-control boundary, materials/waste custody, access condition or quality closeout changes. Record who decided on the control and cite the accepted evidence; do not silently close a deviation because work appears finished. The form provides an OWNER/CUSTODIAN, NEXT REVIEW / EXPIRY and separate contractor, reviewer and dated signature fields so outstanding restrictions transfer across shifts.

WHEN TO STOP OR WITHHOLD RELEASE

An entry is a tracking and accountability tool, not an operator change approval, release from HOLD or permission to connect to plant networks. Where the approved work boundary or authorisation is unclear, stop the affected activity and obtain the actual facility operator's decision and permit variation before resuming.

INDIVIDUAL, PACKAGE OR COMPLETE FILE?

One editable A4 Microsoft Word (DOCX) record costs R12.95. The 260-document Petrochemical Checklist Package contains this same form for R1,559.95 (about R6 per document). For R340 more, the complete 500-page Petrochemical Contractor Safety File currently costs R1,899.95 and includes all 260 records plus the A–AA contractor framework, Section AB and 20 specialist annexures. Price offers can change; only the authorised plant operator may issue actual permits/release.
