AP-H02 — VENTILATION INDEPENDENT WITNESS HOLD & RELEASE
AP-H02 is the original five-gate independent witness hold and release record for underground ventilation fans, ducts and airflow controls. It deals with approved temporary routing, occupied-area gas tests, flame-retardant duct integrity, fan-loss communication and witnessed airflow testing.
WHEN THIS VENTILATION SOURCE APPLIES
Use it before the mine accepts a staged ventilation installation, system change or fan/duct maintenance return. Identify MINE / AREA / LEVEL, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR for the affected mine ventilation district.
