AP-H02 is AVC's original editable Word FIVE-GATE independent inspection, witness and handback hold-point record for contractor interfaces with mine blasting and post-blast re-entry. The first source gate requires full headcount, followed by communication, plant security, residual hazard assessment and formal mine clearance.
WHY VERIFIED POST-BLAST EVIDENCE MATTERS
After a mine blast, a contractor may be eager to return to scaffolding, surveying or equipment while the blast zone is still under control.
