WHERE AND WHY IT IS USED

Use this refinery support-service register before and during activities that depend on authorised personnel, inspected materials, equipment or test evidence. It joins the actual person or asset to the approval and work permit that applies at the operating facility rather than relying on a generic training file disconnected from the workface.

THE ACTUAL PRINTED CONTROL FIELDS

The printed register provides four columns: PERSON / ASSET IDENTIFIER; QUALIFICATION / APPROVAL; PERMIT / PTW REF.; and TEST / EXPIRY / FILE REF. Enter the actual worker, tool, plant support asset or other applicable item and cross-reference the current competency certificate, approval, permit, inspection or test. The form identifies facility/unit/equipment, work order, contractor and shift.

HOW TO COMPLETE AND SIGN IT

Review entries against the issued work order before access, use or inspection, and update them when a licence, permit, test or authorisation expires or changes. File signed certificates, competent-person evidence and test documents behind this register. Assign an OWNER/CUSTODIAN and NEXT REVIEW / EXPIRY date, with named contractor, reviewer and dated signatures to preserve a traceable competence and equipment record.

WHEN TO STOP OR WITHHOLD RELEASE

The register does not qualify a worker or grant plant network, storage or process-area access. If required proof is missing or expired, treat the dependent activity as not ready; obtain the facility's actual authorisation and competent verification before work proceeds.
