255 / AV-H02 — Barrier Reinstatement & Return-to-Service Hold

WHERE AND WHY IT IS USED

Use this hold/release sheet before an operational stores, support-service or digital workface is reinstated or returned to refinery operations. The work may look finished while a plant protection barrier, access credential, chemical custody, isolation or unexpected defect still prevents safe handback. Its six hold points deliberately stop progression pending accepted evidence.

THE ACTUAL PRINTED CONTROL FIELDS

The six source-defined hold steps are: (1) identify and control the affected facility barrier, process boundary or protection system; (2) have the operator screen access, storage compatibility and network security, with no control-system connection without approved OT change; (3) obtain technical inspections, tests and QA evidence; (4) reconcile chemical/waste custody and IT access, clear temporary items, inspect exits and close the support order; (5) retain failed or open items as HOLD / NO RESTART; and (6) verify a clean accounted-for workface before operational authorisation to reinstate.

HOW TO COMPLETE AND SIGN IT

Identify facility, unit/equipment, work order/PTW, contractor and shift. Against every printed HOLD point record the inspection or ITP evidence and the named release authority/date. Enter open defects/NCRs, reinspection and formal release references. Obtain separate contractor and facility-authority signatures, date and time. Do not confuse a contractor's internal completion tick with the operator's actual work permission.

WHEN TO STOP OR WITHHOLD RELEASE

This file is an inspection and decision record; plant isolation, PTW clearance and process restoration remain with the operator. Missing technical tests, an unreconciled control credential or an outstanding critical deviation means affected work remains on HOLD.

INDIVIDUAL, PACKAGE OR COMPLETE FILE?

One editable A4 Microsoft Word (DOCX) record costs R12.95. The 260-document Petrochemical Checklist Package contains this same form for R1,559.95 (about R6 per document). For R340 more, the complete 500-page Petrochemical Contractor Safety File currently costs R1,899.95 and includes all 260 records plus the A–AA contractor framework, Section AB and 20 specialist annexures. Price offers can change; only the authorised plant operator may issue actual permits/release.
