254 / AV-H01 — Stores & Digital Support Inspection Hold and Release

WHERE AND WHY THIS FORM IS USED

Use this workface inspection hold sheet at defined points in refinery stores, support services, controlled materials or digital access work. A contractor may complete a material receipt, equipment test or IT-support action while still awaiting site acceptance; this record prevents progression or handback before the designated facility authority has seen the relevant evidence.

THE SOURCE'S PRINTED CHECKS AND FIELDS

The six actual printed holds are: support work order and access; chemical/material receipt approval; stores segregation and equipment inspection; waste disposal and transfer documentation; control-system/network boundary check; and area/store handover. These are not invented generic milestones: each one is a separate numbered inspection line with columns for evidence/ITP reference and RELEASE BY / DATE.

HOW TO COMPLETE AND FILE EVIDENCE

Complete the heading for facility, unit or equipment, work order and PTW reference, contractor and shift. At each applicable hold, reference the acceptance record, chemical/SDS or delivery paperwork, permit, inspection, network/OT permission or waste manifest. Enter open defect, nonconformance or HOLD reference and separate reinspection/formal release reference. Finish with named contractor and facility-authority signatures, dates and times. A blank release column is not acceptance.

HOLD, RELEASE AND AUTHORITY LIMIT

The source specifically says each identified hold is a controlled work-stop. If material compatibility, OT isolation, testing or disposal evidence is missing, work must remain stopped until the real operator/competent facility authority releases it. This signed form does not issue the permit or control actual process restoration.
