253 / AV-C03 — Process Deviation & Unexpected Condition Stop-Work
WHERE AND WHY THIS FORM IS USED
Use this petrochemical support-trade STOP-WORK checklist when the workface no longer matches the permit, shift conditions, stored chemicals, digital controls or agreed workpack. An unexpected process or SIMOPS condition cannot be handled simply by a supervisor's verbal approval; the operator needs to evaluate the change and permitted restart.
THE SOURCE'S PRINTED CHECKS AND FIELDS
Its eight source-defined checkpoints require: reconfirm plant status and workface boundary with the area owner; evaluate whether stores/digital work could alter plant controls or introduce incompatible chemicals; have the operator screen access, segregation and cybersecurity, without control-system connection absent approved OT change; record process, energy, chemical, ignition or SIMOPS changes and STOP pending reassessment; reconfirm permit and isolation validity; alert neighbouring trades, operations and emergency personnel; file engineering/environmental/quality nonconformance disposition; and document joint contractor-supervisor and operator-area-authority restart or cancellation.
HOW TO COMPLETE AND FILE EVIDENCE
Identify the facility/unit/equipment, contract shift and work order/PTW. Mark the eight rows YES/NO/N/A, justify N/A and enter evidence, open actions and any permit variation. Obtain separate contractor and facility-authority signatures with date/time. Record the outcome of reassessment, not merely the observation that an issue arose.
HOLD, RELEASE AND AUTHORITY LIMIT
The printed HOLD rule treats any unanswered critical point, absent operator permit or unapproved deviation as STOP/HOLD. No informal supervisor-only restart. The form supports change communication and defensible handback but grants no operator authority, new process isolation or waiver.
