252 / AV-C01 — Process-Area Stores & Digital Support Prestart

WHERE AND WHY THIS FORM IS USED

Use this eight-point prestart before contractor stores, supply, logistics, waste or approved digital-support tasks begin inside a live petrochemical or refinery facility. It converts a general stores/service job into a verified site-specific workface with the operator's current process criticality, routes and permits acknowledged before staff or vehicles move.

THE SOURCE'S PRINTED CHECKS AND FIELDS

The actual source checklist requires eight YES/NO/N/A confirmations: facility operator screens the service and process criticality; induction, approved routes and work hours are accepted; flammable/toxic materials have compatible segregated storage; forklifts, vehicles and pedestrians remain separated; waste collection and manifest control are in place; IT/electronic devices do not connect to control systems without permission; fire routes, emergency points and operational panels remain clear; and end-of-shift inventory/location handback will be signed.

HOW TO COMPLETE AND FILE EVIDENCE

Enter the facility, unit/equipment, issued work order, permit-to-work reference, contractor and shift. Check the real items at the workface, use N/A only with a documented basis, and note evidence/open actions or permit variations. Obtain the designated contractor and operator authority names, signatures, date and time. Attach actual induction, material, traffic, permit, waste and digital-access evidence where required.

HOLD, RELEASE AND AUTHORITY LIMIT

An unknown material compatibility, unauthorised network connection, obstructed emergency point or unresolved operator permit cannot be ticked away. The form's HOLD rule requires affected work to stop pending valid authority and re-release; no contractor signature substitutes for plant operating consent.
