WHEN AND WHERE THIS ORIGINAL IS USED
Use this source index during mine workpack preparation and safety-file activation, not as a field permit. Enter MINE / AREA / LEVEL, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR, with the responsible mine ventilation officer identified.
SIX ORIGINAL ANNEXURE ACTIVATION REQUIREMENTS
1. Mine ventilation officer supplies authorised ventilation plan and minimum airflow criteria
2. Fan isolation and electrical backfeed checked without compromising adjacent workfaces
3. Temporary airflow routing, stoppings and door settings approved before disturbance
4. Air quality and gas monitoring arranged for occupied affected districts
5. Duct support, flame-retardant materials and leakage testing accepted
6. Loss-of-fan and re-entry protocol communicated to all exposed crews
