252 / AP-00 — BLASTING SUPPORT WORK-ORDER ACTIVATION AND MINE ACCEPTANCE
AP-00 is the original one-page editable Word work-order activation and mine acceptance record for contractor support adjacent to surface-mine blasting and post-blast re-entry. It contains SIX source control QUESTIONS linking each mine-authorised interface to a real permit/evidence reference and accepting owner.
WHY THESE BLAST SUPPORT SAFEGUARDS MATTER
An approved construction task or supplier work order is not permission to enter a mine blasting zone or conduct an explosive operation. The mine blast officer controls the blasting programme while the contractor must document its actual exclusion route, evacuated people and equipment, warning communication and post-blast hazard awareness.
SIX ORIGINAL WORK-ORDER ACTIVATION QUESTIONS
1. Blast officer retains firing/explosive/clearance control; contractor role defined in order. 2. Controlled exclusion-zone map revision and refuge routes received. 3. Crew, subcontractors and vehicles accounted before evacuation.
4. Radios, blast warning sirens, timetable and boundary marshals tested. 5. Contractor equipment parked and secured outside flyrock/damage zones. 6. Post-blast fume, dust, unstable ground and unexploded-charge exposures recognised.
SOURCE CONTROL FIELDS, REAL EVIDENCE AND AUTHORITY
Its SIX printed fields use a THREE-COLUMN matrix: ACTIVATION / CONTROL QUESTION, DOCUMENT / PERMIT / EVIDENCE REFERENCE and OWNER / ACCEPTANCE. Unlike the pre-start checklist, there are NO Y/N/N-A boxes, measured blast settings or seventh row. Reference current officer instruction, maps, people count, warning checks and safe plant position.
The REVIEW / CLOSE-OUT section separates CONTRACTOR SUPERVISOR / SIGNATURE and MINE RELEASE AUTHORITY / SIGNATURE with DATE / RELEASE REF. An internal work-order signature cannot supersede blast-officer or post-blast zone release authority.
ONE SOURCE WORD SINGLE OR COMPLETE SURFACE MINE FILE?
The identical original purchaser-licensed editable DOCX is ALREADY INCLUDED in the 386-document Surface Mining Checklist Package and complete Surface Mining Contractor H&S File, alongside mine site safety management and specialist annexures. Larger collection purchasers already receive this form. All explosives, blast area clearing and return-to-work decisions remain solely under host mine authority.
