251 / AV-C00 — Support-Trade Activation and Permit Gate
WHERE AND WHY THIS FORM IS USED
Use this activation gate before awarding a live-process workface to a refinery stores, logistics or digital-support contractor. Unlike an ordinary prestart, it confirms whether that trade service should be mobilised at all, who owns the affected plant boundaries and which actual work order and authorisations govern the service.
THE SOURCE'S PRINTED CHECKS AND FIELDS
Its eight numbered acceptance checks establish: a named facility service owner and contract work order; operator screening of support service/process criticality; current induction and approved routes/hours; segregation of flammable or toxic stores; forklift/vehicle/pedestrian separation; waste collection/manifest controls; drawings, technical inspections and permit types agreed with the site issuer; and recorded supervisor plus operator acceptance before mobilisation.
HOW TO COMPLETE AND FILE EVIDENCE
Complete the facility, unit/equipment, actual work order and PTW reference, contractor and shift. Mark each row YES/NO/N/A only against evidence, justify exclusions, identify open items/permit variations and file the relevant operator instructions. The source provides contractor name/sign, separate facility authority signature and date/time fields. Treat this as the release gateway that precedes other Section AV workface forms.
HOLD, RELEASE AND AUTHORITY LIMIT
An unanswered service-owner, criticality or permit gate is a STOP/HOLD; a contractor cannot self-activate work in a live refinery. The tool documents the acceptance boundary and roles but is not the operator's permit, MHI approval or isolated plant handover.
