WHERE THE TOOLBOX TALK APPLIES

Use this NDT and inspection briefing before weld, pressure-system, tank or piping examination in a live petrochemical/refinery facility, especially when a contractor is approaching a witness or hold point. The form highlights the risk that tests can release energy or incorrectly approve safety-critical equipment. Explain the authorised work boundary, current plant condition and inspection method to the actual crew before starting.

FIVE SOURCE-DEFINED INSPECTION CONTROLS

The printed talking points require confirmation of the correct equipment tag and inspection coverage, use of authorised examination methods, radiographic exclusions under the qualified licensee's control, preservation of any inspection HOLD where findings are unacceptable, traceable calibration and recorded test readings, and authorised disposition of repairs before work continues. These controls distinguish an authentic examination record from a generic 'inspection completed' statement.

STOP, DISPOSITION AND RELEASE

Confirm the applicable NDT method, acceptance code, competent examiner and any required radiation licensing before activity. Segregate the operating plant interface and observe the site exclusion arrangements. If an unacceptable indication, missing calibration, mismatched tag or unapproved repair is found, do not conceal it or remove the hold: notify the permit issuer/area authority and wait for documented reassessment and authorised release. Never imply a toolbox attendee signature replaces an NDT witness sign-off.
