248 / AU-C02 — Final Quality & Operations Handover Checklist
WHEN THIS CHECKLIST IS REQUIRED
Use this eight-point closeout checklist after an authorised refinery NDT, test, inspection or repair workpack and before affected equipment or the inspected area is handed back to operating personnel. The original form is titled final quality, decontamination and operations handback; it calls for technical verification, plant-barrier reinstatement, records, housekeeping and host sign-off, not a contractor's informal statement that inspection is finished.
EXACT EIGHT CHECKPOINTS ON THE FORM
The numbered YES/NO/N/A rows verify completed work against approved scope and technical specifications; calibration/certification; hold and witness tests; repair disposition and reinspection; accounting for temporary barriers, tools, material and waste; return of all isolations and permits through operator-controlled processes; acceptance of remaining defects, NCRs and operating restrictions; and final traceable report handback. Where an item is not applicable, record why rather than leaving it blank.
COMPLETION AND EVIDENCE
Identify actual facility, unit/equipment, work order, operator PTW reference, contractor and shift. Mark the evidence status of each original line and enter relevant ITP, certificate, inspection map, test pack, defect disposition or permit variation in the evidence/open-action field. Obtain independent named contractor and facility authority signatures with date/time. Retain the evidence pack behind the form for audit and handback.
HOLD BEFORE RELEASE
The source HOLD rule states that an unanswered critical item, missing plant permit or unapproved deviation requires STOP/HOLD; a contractor tick does not grant operator permission. Do not imply a successful test automatically authorises process reinstatement. Any incomplete reinspection or NCR needs disposition and release by the actual facility authority.
ONE FORM, PACKAGE OR COMPLETE FILE
This individual A4 editable Word document costs R12.95. The Petrochemical 260-Document Checklist Package is R1,559.95 (about R6 per form). The 500-page Petrochemical Contractor H&S File is currently R1,899.95, just R340 above the package, and includes all 260 Word records plus A–AA contractor management, compulsory AB and AC–AV specialist annexures. Actual plant approvals and permits still govern; prices may change.
