WHY AN INSPECTION CHANGE REGISTER MATTERS

A refinery NDT or inspection job may encounter a defect, changed test boundary, unexpected reading, unavailable acceptance certificate or repair decision. Use this editable register to carry such a deviation from discovery through technical control and final operator disposition instead of allowing unresolved findings to disappear across shifts, contractors or test-pack revisions.

FOUR EXACT SOURCE COLUMNS

Each of the seven available record lines links PLANT ITEM / WORK ORDER to DEVIATION / HAZARD, then DECISION / CONTROL / EVIDENCE, and finally RELEASE AUTHORITY / DATE. Use actual plant tags and work orders, explain the finding or changed condition, cite ITP, calibrated test result, engineering disposition, NCR and re-test as applicable, and name the actual authority and release date. A generic 'fixed' entry is not traceable evidence.

WHO MAINTAINS AND SIGNS

Complete the header with facility/unit/equipment, work order or PTW and contractor/shift. Name the OWNER / CUSTODIAN and NEXT REVIEW / EXPIRY so unresolved findings remain active. The table finishes with contractor, reviewer and date/time signatures; attach the signed examination, technical approval or repair evidence to the controlled workpack.

RELEASE DOES NOT FOLLOW AUTOMATICALLY

This register is a decision trail, not an engineering concession, permit or plant startup certificate. A failed inspection, altered NDT method or missing witness approval remains held under the actual workpack and operator processes until competent parties record disposition and authorise the next activity.
