WHEN TO USE THIS REGISTER

Use this competency, authorisation and inspection-evidence register before refinery NDT, examination, calibration and test work, and update it as the workpack progresses. It connects named personnel and test assets to the real qualified-examiner authority, current method approvals, plant permit and traceable calibration rather than relying on a broad claim that the contractor is competent.

WHAT THE ACTUAL FORM RECORDS

The seven-line table has four exact fields: PERSON / ASSET IDENTIFIER; QUALIFICATION / APPROVAL; PERMIT / PTW REF.; and TEST / EXPIRY / FILE REF. Enter the qualified examiner or specific inspection equipment, the relevant approval, actual permit reference and valid certificate or calibrated-test file. Include facility/unit/equipment, work-order reference and contractor/shift at the top, and file signed authority and test records behind it.

CONTROL AND REVIEWS

Identify the register OWNER / CUSTODIAN and NEXT REVIEW / EXPIRY. Check that examination methods and any radiation-related authorisations are appropriate for the work order and current plant status. Update the register as personnel, permits, instruments or approval dates change. Obtain contractor and reviewer names, signatures and date/time in the original sign-off table.

WHAT IT CANNOT AUTHORISE

A name entered in the register does not qualify an examiner, license a radiographic activity or waive an operator PTW. Where competency, calibration, inspection scope or statutory authority is absent/expired, the dependent work remains unreleased until the competent facility and appropriate licensed parties accept it.
