245 / AU-H02 — NDT Barrier Reinstatement & Return-to-Service Hold
WHEN A RETURN-TO-SERVICE HOLD IS NEEDED
Use this six-stage inspection hold after refinery NDT, weld/pressure-equipment examination or repair affecting an operating-facility barrier, process boundary or safety-critical system. Before restoration, confirm the operator's plant protection arrangements, technical inspection and any failed test or repair have been appropriately dispositioned. The sheet expressly treats every identified hold as a controlled work-stop rather than ordinary checklist completion.
ALL SIX SOURCE HOLD POINTS
The printed lines require: (1) identification and control of the affected facility barrier, process boundary or protection system; (2) confirmation of the NDT method, acceptance code, competent examiner and any required radiation licence, while segregating active plant interfaces; (3) approved engineering/technical inspections, testing and QA evidence; (4) HOLD for nonconformity disposition, witness testing and repair/retest, archiving the traceable final certificate; (5) retention of an open action, failed examination or pending retest as HOLD / NO RESTART; and (6) a clean, accounted-for workface with operations authorising reinstatement.
HOW THE HOLD SHEET IS COMPLETED
Fill in facility, unit/equipment, work order/PTW, contractor and shift. Each printed numbered line has EVIDENCE / ITP REF. and RELEASE BY / DATE columns. Cite the inspected plant tags, actual approved NDT and test pack or NCR, record open defects/HOLD references and separate reinspection/formal-release reference. Obtain named contractor and facility-authority signatures, date/time, and file test certificates behind this specific record.
OPERATOR RELEASE IS NOT A CONTRACTOR TICK
The source expressly reserves plant isolation, permits and process restoration for the facility operator. Missing NDT authorization, unresolved defects, failed tests or an incomplete release keeps the activity on HOLD. The editable Word sheet demonstrates the decisions and evidence but does not itself authorise a plant restart.
INDIVIDUAL FORM OR FULL AVC SAFETY SYSTEM?
This editable A4 Word hold sheet is R12.95. The 260-Document Petrochemical Checklist Package is R1,559.95 (approximately R6 per record). The complete 500-page Petrochemical Safety File currently costs R1,899.95, R340 above the package, and contains these same records plus the A–AA management system, compulsory AB and every AC–AV trade annexure. Complete for the actual site; this product is not a plant permit. Prices may change.
