244 / AU-H01 — NDT Inspection, Witness Test & Operator Release Holds

WHERE TO USE THIS INSPECTION HOLD RECORD

Use AU-H01 when a refinery contractor's NDT, QA or inspection activity requires formal witness, examination or acceptance before subsequent work conceals a defect or safety-critical equipment changes state. The form is a hold/release record, not a test certificate; its purpose is to identify who must approve each next stage and the evidence they actually reviewed.

SIX SOURCE HOLD GATES

The actual numbered inspection points are: approved inspection plan; qualified NDT and authorised areas; calibration/certification check; hold and witness test; repair disposition and reinspection; and final traceable report handback. Each gate has EVIDENCE / ITP REF. and RELEASE BY / DATE cells. Use the real inspection/test plan, qualified examiner, certificate and plant tag rather than substituting a generic statement that quality was checked.

COMPLETION, SIGNATURE AND DEFECT CONTROL

Record facility/unit/equipment, work order and PTW, contractor and shift. Enter the applicable ITP reference and the name/date of each authorised release. Where a test fails or a nonconformance is recorded, write the OPEN DEFECT / NCR / HOLD reference and the REINSPECTION & FORMAL RELEASE reference. Obtain independent contractor and facility-authority signatures with actual date/time. File approved witness sheets and examination reports with the workpack.

THE PERMIT BOUNDARY

A contractor cannot remove an operator hold just by completing this Word form. Any unapproved inspection result, missing examination qualification, invalid calibration or failed repair remains a hold until the appropriate competent authority and facility operator permit process release the work; plant isolation and restoration remain operator-controlled.

INDIVIDUAL FORM OR FULL AVC SAFETY SYSTEM?

This editable A4 Word hold sheet is R12.95. The 260-Document Petrochemical Checklist Package is R1,559.95 (approximately R6 per record). The complete 500-page Petrochemical Safety File currently costs R1,899.95, R340 above the package, and contains these same records plus the A–AA management system, compulsory AB and every AC–AV trade annexure. Complete for the actual site; this product is not a plant permit. Prices may change.
