243 / AU-C03 — NDT Process Deviation & Stop-Work Checklist
WHEN TO ACTIVATE THIS STOP-WORK CHECKLIST
Use this record when a refinery NDT test, pressure-system examination, weld inspection or related QA activity encounters a plant condition, test exposure, SIMOPS conflict or defect outside the approved workpack. An altered process boundary or unexpected measurement can invalidate the earlier risk assessment or work permit. The form documents a controlled reassessment and authorised restart—not a casual decision by a contractor supervisor.
THE ORIGINAL EIGHT VERIFICATION POINTS
Confirm the operator has reconfirmed current plant status and workface limits; consider whether testing releases energy or falsely accepts critical equipment; verify the actual NDT method, acceptance code, competent examiner and any radiography licence while segregating active interfaces; document new process/energy/chemical/ignition/SIMOPS issues and stop; reconfirm operator-controlled permits and isolations; notify adjacent trades, operations and emergency responders; record any engineering, environmental or QA NCR disposition; and obtain joint contractor supervisor and operator area-authority documentation for restart or cancellation.
HOW TO COMPLETE AND SIGN
Enter facility, plant unit/equipment, work order, PTW reference, contractor and shift. Mark each numbered YES, NO or N/A field, recording N/A only with supporting justification. Note actual evidence, open actions, permit variations and engineering disposition. Two signature columns identify contractor and facility authority, with date/time. Keep the form with inspection reports, NCRs and permit variation rather than marking a changed condition resolved verbally.
HOLD/RELEASE AUTHORITY
The printed HOLD rule says any unanswered critical item, absent plant permit or unapproved deviation is STOP/HOLD. No informal supervisor-only restart: reinspection, changed NDT arrangements and operating authority's explicit release are required. This editable A4 DOCX is a traceability form, not operator-issued permission to test or resume.

The complete 500-page Petrochemical Contractor H&S File is currently R1,899.95, R340 more than the package, and includes all 260 records, A–AA contractor management, mandatory AB and 20 trade annexures AC–AV. Actual operator permits, inspections and authorisations govern. Current prices may change.
