242 / AU-C01 — NDT & Inspection Process-Area Prestart Checklist

WHERE THIS PRESTART APPLIES

Complete AU-C01 immediately before authorised NDT, weld/tank/pipe examination or quality inspection inside a refinery or other operating process facility. The work permit and operator-issued workpack govern the task; the checklist verifies that examiner competence, test limits, traceability and active plant interfaces have actually been established before the crew starts.

EIGHT SPECIFIC CHECKS PRINTED ON THE DOCUMENT

Record YES/NO/N/A against: a qualified examiner with the correct method and acceptance standard; current instrument calibration and traceability; separately licensed specialist authority for radiography/ionising radiation; accepted plant test boundary, access and SIMOPS controls; verified weld/tank/pipe examination locations and mapping; engineering tracking of defects and NCR disposition; acceptance/release before work is concealed; and indexed final inspection/test plan, certificates and test pack. Each control refers to a real test-workpack element, not a generic site safety tick.

HOW IT IS COMPLETED

Enter the refinery facility, unit/equipment, work order, issued PTW, contractor and shift. Check the actual equipment tag and maps, show certificates/calibration records and current examiner authority, and enter evidence, open actions or permit variation against the checklist. Explain any N/A; obtain named contractor and facility operator signatures with date and time. Link the completed form to the source ITP and reports.

WHEN WORK REMAINS ON HOLD

An unapproved radiographic method, expired calibration, missing operator access, unrecorded test location or open nonconformity is a release blocker. The source HOLD rule makes unanswered critical items, missing permits or deviations STOP/HOLD. Only the actual operator and competent technical authority can authorise affected progression.
