242 / AO-H02 — PRODUCTION DRILL INDEPENDENT INSPECTION AND WITNESSED HANDBACK

AO-H02 is AVC's original five-gate editable Word independent inspection, witness and handback HOLD form for surface mine drill rigs and blast-hole interface work. It concentrates on highwall crest clearance, silica dust controls, hole data, charged-area separation and severe-weather/misfire STOP conditions before mine release.

WHY THIS REAL PRODUCTION DRILL CONTROL MATTERS

A hole can be drilled and documented yet remain a fall/trip exposure if its collar is unprotected, or its work zone can become unsafe when a charged buffer moves. A dusty rig or missed lightning warning is a reason to hold work despite a completed production target. Independent acceptance must distinguish contractor verification from blast-owner permission.

FIVE ORIGINAL INDEPENDENT EVIDENCE GATES

1. Highwall crest setback and flyrock/explosives exclusion communicated to drill crew. 2. Dust extraction and silica respiratory protection maintained at the rig. 3. Hole-collar protection, depth, deviation and water ingress documented for the blasting team.

4. Charged holes and exclusion buffers positively marked and controlled by mine blast authority. 5. Storm/lightning, suspected misfire and blast notification triggers included in work-stop criteria.

USE THE ACTUAL WORD FIELDS AND MINE RELEASE

Enter MINE / PIT / WORK AREA, WORK ORDER / MINE ZONE, DATE / SHIFT / PERMIT and CONTRACTOR SUPERVISOR. The original has precisely FIVE HOLD TRIGGER / EVIDENCE GATE subjects, each with REQUIRED EVIDENCE / VERIFICATION and RESULT / REFERENCE / WITNESS. It does NOT carry a seven-row drilling measurements table. Attach surveyed restrictions and actual status evidence, with a designated mine official's release required.

ALL five gates and identified defects must be resolved before an authorised mine official records RELEASE.

The source distinguishes CONTRACTOR SUPERVISOR / SIGNATURE from MINE RELEASE AUTHORITY / SIGNATURE and DATE / RELEASE REF. A contractor's observations do not overrule the mine operator's blast officer, geotechnical permits, charged-hole exclusion or TARP.
