241 / AU-C00 — NDT Trade Activation & Permit Gate Checklist

WHY THIS ACTIVATION GATE COMES FIRST

Use this reviewer-facing checklist before an NDT or specialist inspection contractor mobilises into an operating refinery workface. Its purpose is to identify the plant service owner, authentic work order and legal examiner competency, and agree test boundaries and the operator's permit requirements. Acceptance at this stage precedes the separate detailed examination prestart and witness/hold sheets.

THE EIGHT PRINTED MOBILISATION GATES

The form verifies a named facility service owner and work order for this trade; the qualified examiner, method and acceptance standard; valid instrument calibration and traceability; separately licensed specialist authority for radiography/ionising radiation; accepted test boundary, access and SIMOPS; weld/tank/pipe inspection locations and mapping; drawings, inspections and required permit types agreed with the site issuer; and recorded supervisor and operator acceptance before mobilisation.

WHAT TO RECORD AND WHO SIGNS

Enter facility and actual unit/equipment, work order/PTW, contractor and shift. Tick each original YES/NO/N/A row only on evidence, recording why a control is not applicable. Use the evidence/open-action/permit-variation field for inspector competency, weld maps, instrument certificate, approvals and site work limits. Both contractor and separate facility-authority signatures with date and time are provided.

MISSING PERMITS ARE A STOP

A contractor may not self-activate an inspection programme in an operating process plant. The printed HOLD rule rejects any unapproved deviation, absent plant permit or unanswered critical acceptance gate. The Word form is a documented activation decision, not a radiographic licence, operator permit or permission to restore process systems.
