AO-F01 is the original five-gate editable Word mine technical HOLD and authorised work release form for production drilling, blast-hole records and unstable drill-pad interfaces. Its first gate is the mine-approved survey pattern and geotechnical pad release.
WHY THIS DRILL WORKFACE CONTROL MATTERS
A rig may be stable and drilling equipment ready while the crest offset, charged-hole exposure or an unprotected collar makes the work zone unsafe. The correct release decision must consider authorised hole positions, stable rig setup, silica controls and recorded hole conditions rather than a contractor's visual all-clear.
FIVE REAL HOLD AND WITNESS GATES
1. Approved survey pattern/geotechnical pad release show authorised hole locations, burden and edge limits. 2. Drill-rig levelling, isolation, mast stability and manufacturer setup validated. 3. Highwall crest setback and mine flyrock/explosives exclusion briefed to drill crew.

The source begins with pattern and pad permission, then rig levelling, highwall/flyrock boundary, respiratory protection and hole collar data. There is no seventh drilling or blast-setting row.
After all five gates, defects and witness records are verified, only the designated mine authority may record RELEASE.
