238 / AT-C02 — Laboratory Sampling Final Quality & Handover

USE THIS BEFORE RETURNING THE SAMPLE POINT TO OPERATIONS

This final quality, decontamination and handback checklist applies after approved refinery sampling or laboratory service activity. Physical sample collection is not a complete closeout if exposure controls, test standards, custody documentation, valve closure and plant permits have not been checked and accepted. The form links the actual sample point and work order to the operator's formal return of the workface.

EIGHT PRINTED HANDOVER CHECKS

The original YES/NO/N/A table requires confirmation of: completed work against approved scope and technical specifications; laboratory exposure controls; calibration/standard checks; chain of custody and test verification; removal or accounting of temporary barriers, tools, materials and waste; return of isolations and permits through the operator's process; accepted remaining defects/NCRs and operating restrictions; and sampling point reinstatement. Each is independently assessed, with N/A documented rather than assumed.

HOW TO RECORD EVIDENCE AND SIGNOFF

Enter facility/unit/equipment, work-order and PTW reference, contractor and shift. Use the evidence/open-action/permit-variation field for sample custody IDs, analytical reports, calibration certificates, waste disposition, valve leak check and any remaining defect. Obtain named contractor and separate facility authority signatures, date and time. Store the editable Word record with the actual sampling and permit closeout pack.

STOP/HOLD BEFORE RESTART

The printed HOLD rule states that unanswered critical items, absent plant permits or deviations not approved by the operator mean STOP/HOLD. Do not treat an analytical result, contractor tick or completed checklist as permission to reopen or restart a sampling point. Release belongs to the facility authority.

CHOOSE THE SINGLE, COMPLETE PACKAGE OR SAFETY FILE

The individual editable A4 Word document is R12.95. The 260-form Petrochemical Checklist Package is R1,559.95 (about R6 per record). The complete 500-page Petrochemical Contractor H&S File currently costs R1,899.95, just R340 above the package, and includes these 260 forms plus A–AA contractor management, compulsory AB and all AC–AV specialist sections. Plant permits and technical release remain with the relevant operator. Prices may change.
