237 / AT-R02 — Laboratory Process Exception & Change Register

WHY LAB AND SAMPLING EXCEPTIONS REQUIRE THEIR OWN REGISTER

Use this refinery laboratory change and closeout register whenever a sample-point condition, unexpected result, reagent incompatibility, analytical deviation or custody issue alters the approved work. It prevents a changed sample status or unresolved technical decision from disappearing between field technicians, the laboratory, the contractor and the facility operator.

THE FOUR ACTUAL SOURCE HEADINGS

Every record line identifies PLANT ITEM / WORK ORDER, DEVIATION / HAZARD, DECISION / CONTROL / EVIDENCE, and RELEASE AUTHORITY / DATE. Capture the true sample-point or instrument tag and work order, describe the sample pressure/chemical or analytical issue, link a corrective action to signed evidence, then identify the authority and release date. The form also has facility/unit/equipment, PTW and shift fields at the top.

MAINTENANCE AND APPROVAL TRAIL

Identify the OWNER / CUSTODIAN and NEXT REVIEW / EXPIRY; unfinished actions must remain open and handed over. Attach applicable instrument calibration, sampling permit, lab verification, retest and contractor/operator decision documents. Obtain contractor/reviewer names, signatures and date/time in the printed closeout table.

WHAT THE REGISTER DOES NOT AUTHORISE

Logging a sampling deviation does not authorise a different sample container, changed operating point or restart after a leak. When process conditions or safety controls change, hold the work for facility reassessment, technical decision and permit variation. The DOCX is a traceability register, not an operator-issued permit.

CHOOSE THE SINGLE, COMPLETE PACKAGE OR SAFETY FILE

The individual editable A4 Word document is R12.95. The 260-form Petrochemical Checklist Package is R1,559.95 (about R6 per record). The complete 500-page Petrochemical Contractor H&S File currently costs R1,899.95, just R340 above the package, and includes these 260 forms plus A–AA contractor management, compulsory AB and all AC–AV specialist sections. Plant permits and technical release remain with the relevant operator. Prices may change.
