236 / AT-R01 — Laboratory Competence & Calibration Evidence Register
WHERE THIS EVIDENCE REGISTER FITS
Use this refinery lab and sampling register before a technician collects process samples, uses specialised analytical equipment or undertakes a permitted test. It provides a link between the actual people and assets, their training/approvals, operator permit references and current inspection or calibration evidence.
WHAT THE WORD FORM ACTUALLY ASKS FOR
The four printed columns are PERSON / ASSET IDENTIFIER; QUALIFICATION / APPROVAL; PERMIT / PTW REF.; and TEST / EXPIRY / FILE REF. A person or equipment entry should have a real identifier, proof of the applicable approval, the operator permit/work order and an actual calibration, training, test or expiry reference. The heading identifies facility/unit/equipment, work order/PTW and contractor/shift.
HOW TO MAINTAIN THE CONTROLLED RECORD
Assign OWNER / CUSTODIAN and NEXT REVIEW / EXPIRY, then keep certificates, sample-system equipment checks, calibration and authority evidence filed against the register. Recheck at shift changes, expired reagents/standards, personnel substitutions or altered test plans. The document has separate contractor and reviewer signature fields with date/time for a traceable review.
WHAT IT CANNOT SUBSTITUTE
An entry is not laboratory accreditation, an authority to sample a live pressurised line or a replacement for training and competent approvals. If a calibration or permit has expired or an operator's permission is missing, the related activity must remain unreleased pending corrected evidence.
CHOOSE THE SINGLE, COMPLETE PACKAGE OR SAFETY FILE
The individual editable A4 Word document is R12.95. The 260-form Petrochemical Checklist Package is R1,559.95 (about R6 per record). The complete 500-page Petrochemical Contractor H&S File currently costs R1,899.95, just R340 above the package, and includes these 260 forms plus A–AA contractor management, compulsory AB and all AC–AV specialist sections. Plant permits and technical release remain with the relevant operator. Prices may change.
