235 / AT-H02 — Sampling Point Reinstatement & Return-to-Service Hold

WHEN TO WITHHOLD REINSTATEMENT

Use this six-stage laboratory/sampling return-to-service hold when a live refinery sample point, analyser or facility barrier is awaiting completion after approved process sampling or lab work. The physical sampling task may be over, but pressure containment, waste, sample custody and the actual valve condition still matter to operations. Each printed hold is a controlled stop until evidenced and released.

ALL SIX HOLD POINTS PRINTED ON THE FORM

Confirm the affected facility barrier, process boundary or protection system is identified and controlled; verify correct sample-point tag, pressure and temperature, approved closed method, PPE and local extraction; obtain approved technical testing and QA evidence; witness sample-valve closure and leak check, reconcile chemical custody and waste and return the analyser/service point safely; retain any open action, failed inspection or retest as HOLD/NO RESTART; and verify clean workface accounting before operations authorises reinstatement.

HOW TO DOCUMENT RELEASE

Complete facility/unit/equipment, PTW/work order and contractor/shift details. For each numbered hold, write the source EVIDENCE / ITP REF. and named RELEASE BY / DATE. Capture the open defect/NCR/HOLD reference and reinspection/formal release reference separately. Obtain actual contractor and facility-authority names, signatures, dates and times, with supporting custody and test records attached.

OPERATOR PERMISSION REMAINS SEPARATE

The original sheet states plant isolation, permits and process restoration remain operator-controlled. A sample valve cannot be returned to service merely because the contractor marks closure complete: unresolved pressure containment, waste discrepancy or leak test means the affected system stays held pending actual facility authorisation.

BUY THE SINGLE OR THE COMPLETE AVC RECORD SYSTEM

This editable A4 Word hold sheet costs R12.95. The 260-form Petrochemical Checklist Package is R1,559.95 (approximately R6 per document). For R340 more, the complete 500-page Contractor Safety File currently costs R1,899.95, with all 260 records plus A–AA professional contractor controls, mandatory Section AB and all AC–AV trades. The source is not a refinery-issued permit. Prices may change.
