233 / AT-C03 — Process Sampling Deviation & STOP-WORK Checklist

WHEN TO USE THIS EIGHT-POINT STOP-WORK RECORD

Complete AT-C03 if a petrochemical sample-taking or laboratory workpack encounters an unexpected plant alarm, uncertain line pressure, hazardous material release, changed sampling method or another condition outside the approved work order. The form is designed to record what changed and why sample work cannot simply continue on the original permit.

EXACT EIGHT PRINTED CHECKS

Check current plant status and workface boundaries with the area owner; assess the source warning of toxic/pressurised process-sample release; verify the tagged sample point, pressure/temperature, closed method, PPE and extraction; document any new process, energy, chemical, ignition or SIMOPS condition and STOP pending reassessment; reconfirm operator-controlled permit and isolation validity without informal restart; notify adjacent trades, operations and emergency personnel; file engineering/environmental/quality NCR disposition where integrity is affected; and record joint permitted restart or cancellation by the contractor supervisor and operator area authority.

HOW TO DOCUMENT EVIDENCE AND SIGNATURES

Fill in facility/unit/equipment, actual work order/PTW, contractor and shift. Use YES/NO/N/A for each printed check; justify N/A and enter permit variations, technical evidence, sample-point issues, actions, leak or exposure records in the provided evidence/open-action field. Obtain independent contractor and facility-authority names/signatures/date/time. Link records to operator decisions rather than creating a parallel contractor-only permit.

WHEN WORK MUST REMAIN STOPPED

The original HOLD rule requires STOP/HOLD where a critical question is unanswered, a plant permit is missing or a deviation is unapproved. A contractor signature cannot authorise continuing to sample a changed line. Only a documented competent reassessment and operator-approved release permits the affected activity to resume.

This exact editable A4 Word document costs R12.95. The Petrochemical 260-Document Checklist Package costs R1,559.95 (about R6 per form). The complete 500-page Petrochemical Contractor H&S File is currently R1,899.95, only R340 more than the package; it includes all 260 Word forms plus A–AA contractor safety framework, compulsory AB and every AC–AV trade annexure. True operator permits and release still govern and current prices may change.
