232 / AT-C01 — Refinery Sampling & Laboratory Prestart Checklist

WHY THIS PRESTART CHECK IS IMPORTANT

Use the actual eight-point sampling prestart before accessing an operating refinery sample point or preparing associated laboratory tests. A seemingly ordinary sample may involve heat, pressure, toxic agents, flammable vapours and custody risks. This record is a workface verification tool under the real operator-issued PTW and sampling workpack, not permission to operate a valve.

EIGHT SOURCE-DEFINED VERIFICATIONS

Verify the correct sample point and current process agent; authorised method and compatible sampling container; protection against toxic, flammable, thermal and splash exposures; chemical-appropriate ventilation/local extraction; reagent compatibility, instrument calibration and inventory; approved transfer of waste and used samples; retained chain of custody and instrument test evidence; and confirmation that the sample point is left closed, labelled and leak free. Mark each original YES/NO/N/A column on actual inspection.

COMPLETING THE CHECKLIST PROPERLY

Enter facility, unit/equipment, work order/PTW, contractor and shift. Inspect the physical point and chemicals, reference the sampling procedure, SDS, container and relevant calibration, and capture open actions or permit variations. Explain N/A against the actual task rather than ticking automatically. Obtain named contractor and facility authority signatures with date/time and attach any sample/custody documentation.

BLOCKERS AND RELEASE

A wrong plant tag, unsuitable container, poor extraction, missing calibration, unverified pressure, incomplete sample-valve check or changed process condition prevents the relevant task. The printed HOLD rule requires STOP/HOLD for an unanswered critical item, absent permit or unapproved deviation until the real operator allows work.

This exact editable A4 Word document costs R12.95. The Petrochemical 260-Document Checklist Package costs R1,559.95 (about R6 per form). The complete 500-page Petrochemical Contractor H&S File is currently R1,899.95, only R340 more than the package; it includes all 260 Word forms plus A–AA contractor safety framework, compulsory AB and every AC–AV trade annexure. True operator permits and release still govern and current prices may change.
