231 / AT-C00 — Laboratory & Sampling Trade Activation Gate
USE BEFORE AUTHORISING THIS TRADE'S MOBILISATION
This reviewer-facing activation checklist sits ahead of the detailed refinery sampling prestart. Its purpose is to confirm that the facility actually requires the nominated laboratory/sampling service, that the correct service owner controls the work order, and that approved methods and exposure controls are ready before contractor personnel mobilise to an operating process area.
EIGHT ORIGINAL CONTRACT ACTIVATION CONTROLS
The printed acceptance gates verify: named facility service owner and legitimate work order; identified sample point and current process agent; approved sample method and appropriate container; toxic/flammable/thermal and splash barriers; chemical-appropriate ventilation/extraction; reagent compatibility, calibration and inventory; drawings, inspection records and permit types agreed with the site issuer; and recorded contractor-supervisor and operator acceptance before mobilisation.
HOW TO COMPLETE THE GATE
Record the facility, actual unit/equipment, PTW/work-order reference, contractor and shift. Mark YES/NO/N/A for each gate against genuine evidence such as sample tags, process-data verification, SDS, container approval, calibrated instrument, ventilation check and issued permit type. Explain any exclusion and record open actions or permit variations. Both contractor and separate facility authority sign and date/time the source form.
WORK CAN BE HELD BEFORE IT EVEN STARTS
The printed HOLD rule prevents release if a critical item is unanswered, the operator's plant permit is absent or a deviation is unapproved. A contractor cannot independently decide that a pressurised chemical sample point is safe or that a lab method is suitable. This DOCX documents the activation decision, not an issued plant permit or lab accreditation.

This exact editable A4 Word document costs R12.95. The Petrochemical 260-Document Checklist Package costs R1,559.95 (about R6 per form). The complete 500-page Petrochemical Contractor H&S File is currently R1,899.95, only R340 more than the package; it includes all 260 Word forms plus A–AA contractor safety framework, compulsory AB and every AC–AV trade annexure. True operator permits and release still govern and current prices may change.
