226 / AN-F01 — CRUSHER CONVEYOR HOLD, RELEASE AND PRODUCTION HANDBACK

AN-F01 is the original editable five-stage HOLD / RELEASE and plant handback form for conveyor, crusher, feeder and chute maintenance at a surface mine. It separates contractor completion from the mine operations owner's permission to proceed to a different energy or production stage.

WHY THIS ORIGINAL MINE SAFETY TOOL MATTERS

Machinery maintenance may involve zero-energy isolation, an unstable trapped-material bridge or removal of large guards. Permitting a first jog test while a person is inside the equipment envelope can cause severe injury, even if the repair itself is satisfactory. This record requires each hold to be cleared before progressing.

FIVE MINE-OWNED HOLD AND RELEASE STAGES

1. Mine operations-owned LOTO and stored-energy proof. 2. Chute/hopper entry authorisation and material clearance. 3. Inspect guards and fitted critical crusher/conveyor components.

4. Operations-owned first jog or trial test-run authorisation. 5. Verify interlocks and emergency stops function before restart.

EXACT EDITABLE WORD FIELDS AND MINE RELEASE

The actual source table is STEP, HOLD / STOP POINT, STATUS (Hold / Release / N/A) and VERIFIER / RECORD (evidence, time, signature). It is NOT a seven-row Y/N/A inspection. Project fields include Mine/site, Area/equipment, Supervisor, Company, Permit/work order and Host-mine work owner. Five genuine hold stages end at interlock and emergency-stop function.

The printed NO RELEASE IF clause explicitly holds the system when a witnessed technical stop, applicable permit or mine acceptance is missing. Separate FINAL CONTRACTOR ACCEPTANCE and HOST-MINE RELEASE names and signature/dates prevent a contractor from self-releasing operating plant.
