225 / AS-H02 — Tanker Transfer Return-to-Service Hold Sheet

WHY THE RETURN-TO-SERVICE HOLD IS NECESSARY

Use this six-stage hold sheet after a refinery bulk delivery or tanker transfer where hoses, loading infrastructure, process barriers or the receiving tank interface need to be returned to normal operations. Stopping the transfer is not the same as reinstating a safe plant condition: valves, hose contents, product manifests, tank identity and operational release must be verified.

SIX ACTUAL PROCESS-BARRIER HOLD POINTS

The source first requires the affected barrier, process boundary or protection system identified and controlled; then independent verification of the product, truck and receiving tank identity, ullage, earthing/bonding and operator transfer communication; approved technical inspections, test and quality evidence; operator-supervised monitored transfer, emergency-stop control, capped valves, reconciled manifests and gate release; any open action/retest/failed inspection retained as HOLD/NO RESTART; and a clean, accounted-for workface released by operations for reinstatement.

HOW TO USE THE PRINTED RELEASE TABLE

Enter the facility/unit/equipment, work order/PTW, contractor and shift. Each numbered control has an EVIDENCE / ITP REF. field and a separate RELEASE BY / DATE field. Refer to authentic connection inspection, tank/vehicle verification, valve condition, manifest, documented test, permit return or gate record. Capture OPEN DEFECT / NCR / HOLD and REINSPECTION & FORMAL RELEASE references, with independent contractor and facility-authority signatures and date/time.

STOP MEANS NO UNAUTHORISED RESTART

The original form explicitly says plant isolation, permits and process restoration are controlled by the operator; this sheet itself is not a permit. A missing product balance, incomplete drain/cap, rejected equipment test or uncertain tank/transfer authority remains held until the actual operating facility accepts and releases the work.

BUY THE INDIVIDUAL FORM OR COMPLETE AVC SAFETY PACKAGE

This original editable A4 Microsoft Word form costs R12.95. The 260-document Petrochemical Checklist Package costs R1,559.95, approximately R6 per form. The 500-page Petrochemical Contractor Safety File is currently R1,899.95, R340 above the package, and includes these 260 forms, the full A–AA contractor framework, mandatory AB and AC–AV specialist annexures. Site permissions are issued by the operator, not this template. Prices can change.
