224 / AS-H01 — Bulk Transfer Inspection & Operator Release Holds

WHERE THIS INSPECTION HOLD SHEET IS USED

Apply this record at defined points in a refinery or tank-farm bulk tanker delivery, loading or unloading sequence. Unlike a general safety prestart, this is a six-step HOLD/RELEASE form: each stage needs genuine evidence and a named competent facility release before affected transfer work progresses.

ALL SIX HOLDS ON THE ORIGINAL WORD FORM

The numbered hold gates are delivery/transfer authorisation; verification of the correct product, receiving tank and capacity; hose, earthing and transfer-equipment inspection; emergency and spill-response readiness; controlled transfer closeout; and final document/gate release. Each item has dedicated EVIDENCE / ITP REF. and RELEASE BY / DATE fields. Tie the line to the actual receiving-tank tag, delivery ticket, hose certificate/inspection, tank ullage, site emergency arrangements and approved transfer controls.

COMPLETING THE EVIDENCE AND RELEASE RECORD

Record facility, plant unit/tank/equipment, work-order/PTW and shift. At every applicable stage cite the approved inspection or transfer evidence and record the named releasing authority and date. If an item fails, enter the OPEN DEFECT / NCR / HOLD reference and separate REINSPECTION & FORMAL RELEASE reference. Finish with the contractor and facility-authority signatures, date and time. The editable form is for the actual job and should remain with the transfer pack.

NO CONTRACTOR SELF-RELEASE

The printed instruction treats each identified hold as a controlled stop; the source reserves plant isolation, PTW and restoration to the facility operator. Unverified tank ullage, wrong destination, noncompliant hose/earth, missing spill controls or absent release means do not open the next stage. An internal contractor signoff alone never authorises flow.

This original editable A4 Microsoft Word form costs R12.95. The 260-document Petrochemical Checklist Package costs R1,559.95, approximately R6 per form. The 500-page Petrochemical Contractor Safety File is currently R1,899.95, R340 above the package, and includes these 260 forms, the full A–AA contractor framework, mandatory AB and AC–AV specialist annexures. Site permissions are issued by the operator, not this template. Prices can change.
