223 / AS-C03 — Bulk Transfer Deviation & STOP-WORK Checklist

USE THIS FORM WHEN THE APPROVED TRANSFER CHANGES

Complete this eight-point STOP-WORK checklist where a petrochemical tanker unloading/loading operation develops an unexpected tank condition, overfill risk, suspect product, changed permit, hose failure, hazardous goods conflict or affected SIMOPS. The original source explicitly identifies bulk transfer to the wrong destination or tank overfill as a hazard; it requires an authorised reassessment and decision before any affected activity restarts.

THE EIGHT SOURCE CHECKPOINTS

Reconfirm the current plant status and workface boundary with the operator area owner; consider the wrong-destination/overfill hazard; independently verify the product, truck/tank identity, ullage, earth/bond and transfer-authority communications; record new process, energy, chemical, ignition or SIMOPS conditions and stop; reconfirm operator-controlled permit and isolation validity; advise adjacent trades, operations and emergency personnel; file engineering/environmental/quality nonconformity disposition; and jointly document permitted restart or cancellation through contractor supervisor and operator area authority.

WHAT TO MARK AND HOW TO SIGN

Enter facility/unit/tank/equipment, work order/PTW, contractor and shift. Each of the eight lines contains Y, N and N/A fields. Treat a negative or uncertain critical item as unresolved; document permit variation, test results, transfer ticket/manifest, exposure/incident evidence and open action. Explain any N/A with actual reasons. The source has separate contractor and facility-authority signature fields, with date/time.

HOLD UNTIL A REAL PERMITTED RESTART

The printed HOLD rule requires STOP/HOLD if a critical item is unanswered, plant permit missing or deviation unapproved. Do not substitute a supervisor's approval or this checklist for the tank-farm operator's authorisation. The document records the change, escalation and verified release decision.
