WHEN THIS PRESTART IS REQUIRED

Complete AS-C01 at the actual tank-farm loading/unloading or bulk tanker delivery workface before starting product flow. The operator-issued permit and work package govern the task. This checklist makes the contractor and plant receiver check vehicle documentation, correct receiving tank, hose condition, exclusion arrangements and the controlled transfer and disconnect sequence before the work begins.

The printed YES/NO/N/A table asks for: carrier, vehicle, driver and dangerous-goods documentation; vehicle containment, brake/earth and applicable bonding controls; correct product, receiving tank and available capacity; hose rating, chemical compatibility and connection method; vehicle/pedestrian exclusion and emergency access; overfill and spill-response interfaces; loading/unloading under the authorised site operator; and witnessed transfer completion, disconnection and manifest reconciliation. HOW TO CHECK THE WORKFACE AND RECORD RESULTS

Identify facility/unit/tank/equipment, real work-order and permit-to-work reference, contractor and shift. Physically inspect the tanker route, hose connection, receiving-tank tag and emergency controls; check delivery ticket and available ullage with the receiving operator. Tick Y/N/N/A only on actual evidence, justify any N/A and record open action or permit variation. Obtain distinct contractor and facility-authority signatures, date and time, in the Word record.

A FAILED CRITICAL CHECK MEANS NO TRANSFER

If the vehicle documentation, tank/product match, capacity, earth/bond, connection integrity or transfer authority is uncertain, retain STOP/HOLD pending plant assessment and approval. The form expressly says unanswered critical items, missing permits or unapproved deviations cannot be bypassed by a contractor tick.
