218 / AR-C02 — Effluent & Drainage Final Handover Checklist
WHEN THE EFFLUENT HANDOVER IS REQUIRED
Complete this eight-point refinery wastewater closeout after an approved pit, drain, temporary bypass or effluent pumping workpack, before the facility operator accepts the restored drainage system. Finishing the repair or decontamination is not enough: temporary hoses, isolations, waste manifests, leak checks and operating restrictions must be accounted for.
EIGHT ORIGINAL QUALITY AND RETURN CHECKS
The printed YES/NO/N/A table verifies work against approved technical scope; release of temporary pumping and bypass arrangements; leak, pressure and cleanliness checks; accepted waste manifests; removal/accounting of temporary barriers, tools, materials and waste; return of isolations and permits through the operator's process; documentation and acceptance of open defects/NCRs and operating restrictions; and final sump/drain functional handover. Record why a control is genuinely not applicable.
HOW TO COMPLETE THE EVIDENCE
Enter facility, actual sump/drain/unit/equipment, work order/PTW reference, contractor and shift. Against the printed line cite pump test, diversion removal, wash/inspection, wastewater sampling or disposal evidence, restoration of drain path and operator verification. Use the EVIDENCE / OPEN ACTION / PERMIT VARIATION field. The contractor and facility authority each have a signed/date-and-time field in the original editable A4 Word document.
WHEN RELEASE MUST BE WITHHELD
The printed HOLD rule is explicit: unanswered critical checks, absent plant permits or an unapproved deviation are STOP/HOLD, and a contractor tick is not operator permission. An unapproved discharge route, unreconciled contamination or failed sump function cannot be closed informally; the responsible facility person controls acceptance.
INDIVIDUAL FORM OR COMPLETE AVC FILE?
This editable A4 Word document is R12.95. The 260-Document Petrochemical Checklist Package costs R1,559.95 (approximately R6 per record). The complete 500-page Petrochemical Contractor Safety File is currently R1,899.95, just R340 more than the package and including all 260 records plus A–AA contractor management, compulsory AB and AC–AV specialist annexures. Only the operator issues actual permits and release; prices may change.
