217 / AR-R02 — Wastewater Process Deviation & Closeout Register
WHY DEVIATIONS MUST BE RECORDED
Use this controlled wastewater/effluent register when a temporary bypass, drain condition, contaminated sludge, pump or disposal arrangement differs from the approved refinery workpack. A change may alter discharge destination or environmental, chemical and biological risk. The original register gives the contractor and reviewer a traceable decision trail through reassessment and formal closeout.
FOUR ACTUAL REGISTER HEADINGS
Each row links PLANT ITEM / WORK ORDER to DEVIATION / HAZARD, DECISION / CONTROL / EVIDENCE and RELEASE AUTHORITY / DATE. Identify the real drain/sump, diversion/pump or discharge work order, describe the condition and consequence, cite inspection, sampling, manifest, test or operator instruction, then record the authority and date accepting the control. Do not simply write 'resolved' with no evidence.
HOW TO MAINTAIN AND SIGN THE RECORD
Complete facility/unit/equipment, work order/PTW and contractor/shift at the top. Assign OWNER / CUSTODIAN and NEXT REVIEW / EXPIRY; open actions stay traceable across shifts or changes in disposal arrangements. File issued permits, water-quality test results, approved routing drawings, nonconformity and discharge records behind the table. Separate contractor and reviewer signatures/date-time are provided.
A REGISTER IS NOT A DISCHARGE CONSENT
Logging a wrong destination, pump isolation error, contamination issue or environmental release does not authorise the new route. The plant operator and applicable environmental authorities retain actual control over changes. Stop the affected work when permitted boundaries are not established and obtain documented decision and release.
INDIVIDUAL FORM OR COMPLETE AVC FILE?
This editable A4 Word document is R12.95. The 260-Document Petrochemical Checklist Package costs R1,559.95 (approximately R6 per record). The complete 500-page Petrochemical Contractor Safety File is currently R1,899.95, just R340 more than the package and including all 260 records plus A–AA contractor management, compulsory AB and AC–AV specialist annexures. Only the operator issues actual permits and release; prices may change.
