216 / AR-R01 — Effluent Work Competence & Inspection Register

WHERE THIS REGISTER IS USED

Use the original AR-R01 competence, authorisation and inspection-evidence register before refinery drain, effluent pit, pumping, bypass, sampling or contaminated-sludge handling tasks. It connects the actual worker and site equipment to current approvals, operator permits and inspections rather than treating general induction as permission for pit entry or pump isolation.

FOUR EXACT SOURCE FIELDS

The printed register headings are PERSON / ASSET IDENTIFIER, QUALIFICATION / APPROVAL, PERMIT / PTW REF. and TEST / EXPIRY / FILE REF. Record the nominated competent person, pump, electrical isolation/test asset or associated equipment; enter the actual approval and facility PTW, relevant inspection, test, training or expiry reference. The top fields identify facility/unit/equipment, work order and shift.

MAINTAINING VALID EVIDENCE

Assign the OWNER / CUSTODIAN and NEXT REVIEW / EXPIRY so expired competence, inspection or permits can be caught before work proceeds. File the related proof, such as pump test, electrical isolation verification, environmental/handling approval, competence record or equipment certificate. The original A4 Word register provides contractor/reviewer names, signatures and date/time.

NOT A SUBSTITUTE FOR A PERMIT

An entered worker or pump is not thereby certified or accepted for a hazardous drainage task. Confined-space/engulfment assessment, chemical identity and automatic pump-start isolation still require actual competent and operator approval. If proof or permit is missing or expired, treat the activity as unreleased.

INDIVIDUAL FORM OR COMPLETE AVC FILE?

This editable A4 Word document is R12.95. The 260-Document Petrochemical Checklist Package costs R1,559.95 (approximately R6 per record). The complete 500-page Petrochemical Contractor Safety File is currently R1,899.95, just R340 more than the package and including all 260 records plus A–AA contractor management, compulsory AB and AC–AV specialist annexures. Only the operator issues actual permits and release; prices may change.
