216 / AM-R04 — MINE HEIGHT ACCESS ASSET TRACEABILITY AND DEFECT CLOSURE

AM-R04 is the original editable Word ASSET TRACEABILITY and DEFECT CLOSURE evidence register for surface mine scaffolding, specialist access and height work. It starts with the mine area owner's release of scaffold removal and restoration of exposed barriers, then traces the conditions on which that handback relies.

WHY THIS ORIGINAL MINE HEIGHT CONTROL IS NEEDED

Scaffolding may be physically dismantled while anchor assets, access barriers or the exclusion of falling objects have not been properly reinstated. A tag without the current permit, an incompatible fall-arrest arrangement or a missing rescue plan remains a serious open defect. Asset history must be tied to a real mine work order and closeout authority.

SEVEN SOURCE EVIDENCE AND CLOSURE ITEMS

1. Mine area owner releases scaffold removal; restores barriers and plant guards. 2. Verify fall-arrest equipment compatibility, free-fall clearance and edge protection. 3. Recheck scaffold tags and modification permits at each shift. 4. Confirm retrieval routes, rescue access time, mine response and communication.

5. Dropped-object isolation protects haul roads, beltways and people below. 6. Wind, lightning and blast vibration accounted for on platforms. 7. Barricade highwall edges, surge bins, ore passes, conveyors and tanks.

SOURCE WORD FIELDS AND APPROVAL RECORD

Seven source evidence rows run BACKWARD from mine owner release to edge barricading. Columns are MINING-SPECIFIC ITEM / EVIDENCE TO TRACK, DOCUMENT / RESULT / MEASURED VALUE, and DATE / AUTHORITY / STATUS. Record scaffold tag, anchor record, rescue drill and handover refs, plus actual defect/NCR/reinspection retention reference and mine/contractor signatures.

Keep source photos, certificates, mine-issued permit references, asset IDs and authorised decisions with the completed form. Blank entries or an unresolved fall hazard cannot substitute for clearance.

SINGLE EDITABLE FORM OR COMPLETE SURFACE MINING SAFETY FILE?

This identical original purchaser-licensed editable DOCX is ALREADY INCLUDED in the complete 386-document Surface Mining Checklist Package and in the full Surface Mining Contractor H&S File. Those comprehensive collections contain this Single alongside other site forms, specialist annexures and management controls; buying the larger set removes the need to purchase this original again. Mine access and rescue authorisations remain controlling.
