215 / AR-H02 — Wastewater Drain Reinstatement & Return Hold
WHY A FORMAL REINSTATEMENT HOLD IS NEEDED
Use this six-stage refinery effluent hold sheet after temporary pumping, sump work, contaminated-water diversion or drain repair before restoring normal wastewater operations. Removing a pump or bypass does not establish safe return: drain route, chemical/biological contamination, automatic starts, valve function and environmental acceptance must be checked by the responsible facility authority.
THE SIX EXACT HOLDS IN THE SOURCE
The numbered work-stops are: identify and control the affected facility barrier, process boundary or protection system; verify the drain network destination, chemical/biological content, pump auto-start isolation and confined-space/engulfment risks; secure approved engineering inspections, test and quality evidence; witness removal of the temporary bypass, final effluent/water-quality verification and correct valve/pump operation; keep any open action, retest or failed inspection at HOLD/NO RESTART; and account for the workface before operations authorises reinstatement.
EVIDENCE, REINSPECTION AND SIGNATURES
Enter facility/unit/equipment, original work order/PTW and contractor/shift. Each hold row has EVIDENCE / ITP REF. and RELEASE BY / DATE fields. Cite actual isolation, sampling/water-quality, pump or valve test, approved restoration and disposal evidence, not generic completion. Enter OPEN DEFECT / NCR / HOLD and separate REINSPECTION & FORMAL RELEASE references, then obtain both contractor and facility-authority signatures/date-time.
RESTART ONLY UNDER FACILITY CONTROL
The printed warning reserves process restoration, plant isolation and permits for the operator; this sheet is not a permit. An open pit hazard, bypass still connected, failed pump/valve test, unapproved discharge or unresolved contamination means no automatic restart even where a contractor has signed.
