214 / AR-H01 — Effluent Inspection & Operator Release Holds

WHERE THIS INSPECTION HOLD APPLIES

Use this six-gate refinery wastewater hold-point record where effluent drains, sumps or temporary pumping/bypass arrangements require inspection and operator acceptance before the next work stage. These are controlled stop/release decisions, not checkboxes granting a contractor the right to change drainage routing or start pumps.

SIX ORIGINAL INSPECTION GATES

The source names the drain/effluent content briefing; isolation and entry/edge controls; temporary pumping and bypass release; leak/pressure and cleanliness checks; acceptance of waste manifests; and sump/drain function handover. Every gate has a dedicated EVIDENCE / ITP REF. field and RELEASE BY / DATE. Use the actual sump/drain asset ID, approved route, signed test or sampling result, pit protection, waste tracking and competent operator release.

HOW TO COMPLETE AND CLOSE OPEN HOLDS

Fill in facility, unit/equipment, work order/PTW, contractor and shift. At each printed hold cite real inspection/ITP proof and the responsible releasing person. If chemical identity, pump isolation, edge protection, waste acceptance, bypass removal or a leak test is outstanding, enter its OPEN DEFECT / NCR / HOLD reference, and only then record a distinct REINSPECTION & FORMAL RELEASE reference after acceptance. Contractor and facility authority sign with dates and times.

NOT A SUBSTITUTE FOR THE OPERATOR PERMIT

The original hold form states operator plant isolation, PTW and process restoration remain controlled by the facility; its release column cannot substitute for the actual authority. Without valid permission or accepted evidence, hold the affected activity and maintain process safety and environmental controls.
