213 / AR-C03 — Wastewater Deviation & STOP-WORK Checklist
WHEN THIS STOP-WORK CHECKLIST MUST BE USED
Use this eight-point record when refinery effluent work finds an unexpected chemical, flammable wastewater, disputed drain routing, unisolated pump, sump atmosphere, failed bypass or changed discharge conditions beyond the authorised work order. The source warns of flammable or toxic effluent entering an open drain, sump or temporary bypass. The purpose is to document reassessment rather than allow an informal continuation.
THE EIGHT PRINTED STOP-WORK CHECKS
Reconfirm current plant status and workface boundary with the area owner; recognise the toxic/flammable effluent exposure into drains or bypasses; verify drain-network destination, chemical/biological content, pump auto-start isolation and confined-space/engulfment risk; identify any new process, energy, chemical, ignition or SIMOPS condition and stop; reconfirm valid operator permits and isolations; notify neighbouring trades, operations and emergency response personnel; file relevant engineering/environmental/quality nonconformity disposition; and document restart or cancellation jointly by contractor supervisor and operator area authority.
HOW TO USE THE PRINTED Y/N/N/A TABLE
Identify facility, drain/sump/unit/equipment, work order/PTW and shift. For each of eight rows tick Y/N/N/A and document an actual N/A justification. Use the EVIDENCE / OPEN ACTION / PERMIT VARIATION field for sampling, drain map, spill/contamination, pump isolation and operator instructions. Both contractor and facility authority sign with date/time when the disposition is established.
DO NOT RESTART WITHOUT OPERATOR RELEASE
The source HOLD rule requires STOP/HOLD for unanswered critical items, missing plant permits or unapproved deviations. A contractor's verbal approval cannot authorise a changed effluent route, pump energisation or entry to a contaminated pit. Retain the hold until the actual area authority has reassessed and released the work.
