212 / AR-C01 — Effluent Drainage Process-Area Prestart
WHEN THIS PRESTART MUST BE COMPLETED
Use this refinery prestart immediately before authorised work on an effluent line, contaminated sump or drainage/temporary pumping system. A drain may contain flammable, toxic or biologically hazardous material even when it looks like ordinary water. The source form requires actual process identification, drain ownership, pit controls and restoration planning within the facility's issued work order.
ALL EIGHT CHECKS IN THE SOURCE
The actual YES/NO/N/A table confirms effluent chemicals and biological hazards; operator-identified drain ownership and prohibited discharges; pit or confined entry, falls and drowning prevention; pumping, pressure, automatic start and electrical isolation; approved bypass/diversion and spill contingency; water-quality sampling requirements; controlled waste solids/sludge disposal; and accepted leak testing and restoration of drainage. These controls are independently ticked, not assumed from a general induction.
HOW TO USE THE WORKFACE AND EVIDENCE FIELDS
Complete facility/unit/equipment, genuine work order/PTW, contractor and shift. Inspect the actual drain/sump route and pump isolation, identify effluent hazards using operator process information, then record check results. Explain any N/A, cite the discharge and sampling requirements and enter open actions/permit variations. Obtain separate contractor and facility-authority names, signatures and date/time before proceeding.
A CRITICAL FAILURE MEANS STOP/HOLD
The printed source rule requires STOP/HOLD for any unanswered critical control, absent plant permit or unapproved deviation. An uncertain drain destination, uncontrolled automatic pump, unassessed engulfment or illegal discharge cannot be cleared by a contractor tick. Operator-authorised reassessment and release govern restart.

This editable A4 Word document is R12.95. The 260-Document Petrochemical Checklist Package is R1,559.95 (approximately R6 each). The complete 500-page Petrochemical Contractor Safety File currently costs R1,899.95, R340 above the package, and includes these same forms plus A–AA management, compulsory AB and all specialist AC–AV annexures. The facility operator retains real permits and release control; prices may change.
