211 / AR-C00 — Wastewater Trade Activation & Permit Gate
WHY ACTIVATION PRECEDES EFFLUENT WORK
Use this reviewer-facing refinery wastewater activation gate before mobilising a drain, sump, pump, treatment, bypass or sludge-handling contractor into an operating facility. It confirms the real service owner and task, the nature of the effluent, plant access and permitted discharge or diversion controls before contractor personnel begin, rather than treating activation as automatic after appointment.
EIGHT ACTUAL MOBILISATION CONTROLS
The source checklist requires a named facility service owner and relevant authorised work order; chemical and biological hazards of the effluent stream; operator confirmation of drain ownership and prohibited discharges; confined-entry, pit-access, fall and drowning assessment; pump/pressure/automatic start and electrical isolation control; approved bypass/diversion and spill contingency; required drawings, inspections and permit types agreed with the site issuer; and recorded contractor supervisor plus facility operator acceptance before mobilisation.
HOW TO RECORD GENUINE AUTHORITY
Enter facility/unit/equipment, work order and PTW reference, contractor and shift. Record YES/NO/N/A for each of eight source questions; give reasons for a genuine N/A and attach site drainage mapping, process hazard confirmation, isolation, workplan and environmental instructions to the open-action/evidence area. The original A4 Word sheet has contractor and facility-authority signatures and date/time.
DO NOT MOBILISE ON AN UNCLEAR BOUNDARY
The printed HOLD rule bars release where critical controls are unanswered, the plant permit is missing or deviation is unapproved. A contractor cannot assign a discharge destination, enter an effluent pit or energise a pump without the actual facility's permission. This template documents the activation evidence; it does not issue a permit.

This editable A4 Word document is R12.95. The 260-Document Petrochemical Checklist Package is R1,559.95 (approximately R6 each). The complete 500-page Petrochemical Contractor Safety File currently costs R1,899.95, R340 above the package, and includes these same forms plus A–AA management, compulsory AB and all specialist AC–AV annexures. The facility operator retains real permits and release control; prices may change.
