210 / AQ-TB-02 — Hot Process Insulation Removal Toolbox Talk
WHERE AND WHY THIS RECORD IS USED
Insulation removal in an operating refinery can expose hot process surfaces, previously concealed leaks, corrosion under insulation and potentially regulated asbestos; an otherwise routine coating/insulation task can affect containment and personnel contact protection.
THE ACTUAL WORKFACE APPLICATION
Before disturbing insulation around a live or isolated pipe, vessel or process service, brief the actual crew against the facility-approved job scope, material assessment, equipment tag and current permit restrictions.
FIVE WORKFACE TALKING POINTS AND DEMONSTRATIONS
1. Confirm insulation contains no regulated asbestos before disturbance.; 2. Verify hot surface and process leak exposure risks.; 3. Collect corrosion findings and photograph concealed pipe condition.; 4. Obtain inspection release before closing repaired insulation.; 5. Restore personnel touch protection and weather seals.
HOW TO COMPLETE IT AND KEEP EVIDENCE
Demonstrate the suspect surface, inspect permitted access and corrosion records, record photographs and explain who accepts exposed substrate before the insulation is closed. Confirm hazardous-area classification, coating SDS, containment, CUI inspection access and control of drift toward process air intakes. Complete the actual facility/unit/equipment, operator work-order/PTW and contractor/shift heading. Ask which hazard might change, who controls the isolation and what proves permission to restart. Record action/questions, facilitator name/signature/date/shift and up to 16 employee ID and attendee signatures. File the original editable A4 Word talk with the actual workpack; it documents a briefing rather than issuing a permit.
WHAT MUST STOP THE WORK
If insulation composition, process-leak condition, temperature, containment or inspection release is uncertain, stop the affected task, notify the operator permit issuer/area authority and seek documented reassessment and release.
INDIVIDUAL FORM, PACKAGE OR COMPLETE SAFETY FILE
One individually editable A4 Microsoft Word form costs R12.95. The 260-document Petrochemical Checklist Package is R1,559.95, about R6 per record. The complete 500-page Petrochemical Contractor Safety File is currently R1,899.95, just R340 above the package, and includes all 260 forms plus A–AA management, compulsory AB and all specialist AC–AV trade annexures. The operator controls permits and actual releases; prices may change.
