206 / AQ-R01 — Coating Competence & Inspection Evidence Register

WHEN AND WHY TO USE IT

This register links actual source evidence with people or plant items and remains open for review until accepted. Before a contractor uses solvent systems, abrasive blasting, fireproofing or insulation removal around operating equipment, the facility needs evidence of the actual qualified personnel, approved materials, calibrated measuring tools and applicable permits. This is a controlled link between the person or asset and its inspection proof.

THE ACTUAL PRINTED VERIFICATION / EVIDENCE FIELDS

The four printed columns have a row for each case or item; the original provides OWNER/CUSTODIAN, NEXT REVIEW/EXPIRY and two signoff areas. 1. PERSON / ASSET IDENTIFIER; 2. QUALIFICATION / APPROVAL; 3. PERMIT / PTW REF.; 4. TEST / EXPIRY / FILE REF.

HOW TO USE IT PROPERLY

Enter actual crew names or instrument IDs and link qualifications, site authorisations, hot-work/chemical work permissions, thickness or test-equipment calibration, dates and file locations. Assign OWNER/CUSTODIAN and NEXT REVIEW/EXPIRY, attach certificates and obtain contractor and reviewer signatures with date/time. The completed source form includes plant/facility identifiers, permit/work order, contractor/shift and separate contractor and reviewer signatures.

WHAT HOLDS THE WORK

No register entry grants a trade qualification or operating-site permit. Missing competent approval, expired calibration, asbestos-screening evidence or plant PTW keeps affected work unreleased.
